Summary
Isavuconazole is a moderate CYP3A4 inhibitor and may modestly raise fludrocortisone exposure, increasing mineralocorticoid effects such as hypokalemia and fluid retention. Corticosteroids may also add to infection risk in patients treated for invasive fungal disease, although fludrocortisone has weak immunosuppressive activity.
What this interaction means in plain words
Fludrocortisone and Isavuconazonium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fludrocortisone belongs to Endocrine; Isavuconazonium belongs to Anti Fungals.
Why it happens
Isavuconazole inhibits CYP3A4, reducing fludrocortisone metabolism.
What to do
Monitor blood pressure, potassium and fluid status, and reduce the fludrocortisone dose if excess mineralocorticoid effects occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
