Summary
Isavuconazole, the active moiety, is a moderate CYP3A4 inhibitor and raises cisapride exposure. Because cisapride is a CYP3A4 substrate with serious QT risk, higher levels may provoke torsades de pointes. Cisapride does not meaningfully affect isavuconazole.
What this interaction means in plain words
Cisapride and Isavuconazonium are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Cisapride belongs to Gastrointestinal Tract; Isavuconazonium belongs to Anti Fungals.
Why it happens
Inhibition of CYP3A4-mediated cisapride metabolism by isavuconazole, increasing cisapride concentrations and QT prolongation.
What to do
Avoid the combination; use an alternative prokinetic or antifungal.
