Summary

Corticosteroids given with neuromuscular blockers, particularly during prolonged ICU use, have been associated with acute quadriplegic myopathy. Fludrocortisone has predominantly mineralocorticoid activity, so the risk is likely lower than with glucocorticoids, and it may also cause hypokalemia that can alter blockade.

What this interaction means in plain words

Fludrocortisone and Pancuronium have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Fludrocortisone belongs to Endocrine; Pancuronium belongs to Musculoskeletal.

Evidence level: Low

Why it happens

Proposed additive muscle toxicity (steroid myopathy with prolonged denervation-like effect of blockade), plus potassium shifts altering neuromuscular transmission.

What to do

Monitor serum potassium and muscle strength if prolonged blockade is needed. Use the lowest effective duration of pancuronium.
