What this interaction means in plain words

Fludrocortisone and Rifapentine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fludrocortisone belongs to Endocrine; Rifapentine belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 by rifapentine increasing corticosteroid clearance.

What to do

Monitor blood pressure, sodium and potassium closely and increase the fludrocortisone dose as needed. Reassess the dose after rifapentine is stopped.
