Summary
Ritonavir is a strong CYP3A4 inhibitor and P-glycoprotein inhibitor that can raise corticosteroid exposure. This may cause fluid retention, hypertension, hypokalemia and, with prolonged use, Cushingoid effects and adrenal suppression.
What this interaction means in plain words
Fludrocortisone and Ritonavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fludrocortisone belongs to Endocrine; Ritonavir belongs to Anti Virals.
Why it happens
Inhibition of CYP3A4 and P-glycoprotein by ritonavir reduces fludrocortisone clearance.
What to do
Monitor blood pressure, edema and serum potassium, and lower the fludrocortisone dose if signs of excess appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
