Summary

Corticosteroids and NSAIDs both damage the gastrointestinal mucosa, and using them together raises the chance of gastric irritation, ulceration and bleeding. Fludrocortisone also causes sodium and fluid retention, which may blunt blood pressure control and worsen edema when combined with an NSAID.

What this interaction means in plain words

Fludrocortisone and Sulindac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fludrocortisone belongs to Endocrine; Sulindac belongs to Musculoskeletal.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic mucosal injury from NSAID prostaglandin inhibition and corticosteroid effects, plus additive sodium and fluid retention.

What to do

Use the lowest effective sulindac dose for the shortest time and consider gastroprotection with a proton pump inhibitor in at-risk patients. Monitor for GI bleeding symptoms, edema and blood pressure.

What to ask your pharmacist

  • Should I take them at different times of day?
