Summary

Flumazenil reverses the effects of benzodiazepines such as temazepam by blocking the benzodiazepine site of the GABA-A receptor. In patients who take temazepam regularly, this can trigger acute withdrawal, including agitation and seizures. Seizure risk is higher in long-term users and in mixed overdoses involving proconvulsant drugs.

What this interaction means in plain words

Flumazenil and Temazepam can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Flumazenil belongs to Central Nervous System; Temazepam belongs to Central Nervous System.

Evidence level: High

Why it happens

Flumazenil is a competitive antagonist at the benzodiazepine site of the GABA-A receptor, displacing temazepam and precipitating withdrawal in dependent patients.

What to do

Avoid flumazenil in patients with chronic benzodiazepine use, physical dependence, or seizure disorders, and in mixed overdoses with proconvulsants. If it is needed, give small titrated doses with seizure precautions and monitor for resedation.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
