Drug interactions
fluoxetine + ibuprofen
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Additive bleeding risk (GI bleeding/ulceration and other hemorrhage) when SSRI is combined with an NSAID; risk higher in older adults, prior ulcer/bleed, high NSAID doses, or concurrent anticoagulants/antiplatelets.
Mechanism of action
Pharmacodynamic: SSRI-induced platelet serotonin depletion impairs aggregation; NSAID inhibits COX-1 and compromises gastric mucosa/platelet function → increased bleeding.
Clinical management
Avoid combination when possible or use acetaminophen. If needed, use lowest NSAID dose/shortest duration; consider PPI gastroprotection in at-risk patients; monitor for GI bleeding, bruising; check Hgb/Hct if symptoms.
