Summary
Voclosporin is a calcineurin inhibitor that can cause dose-related nephrotoxicity, and NSAIDs reduce renal prostaglandin-dependent perfusion. Together they raise the risk of acute kidney injury, which is important in lupus nephritis patients with already compromised kidneys.
What this interaction means in plain words
Flurbiprofen and Voclosporin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Flurbiprofen belongs to Musculoskeletal; Voclosporin belongs to Miscellaneous.
Why it happens
Additive renal hemodynamic impairment from afferent arteriolar constriction by voclosporin and prostaglandin inhibition by the NSAID.
What to do
Avoid NSAIDs where possible; if required, use short courses and monitor eGFR, serum creatinine and blood pressure.
