Summary

Voriconazole inhibits CYP3A4 and can increase systemic exposure to fluticasone, which normally has very low systemic availability. Prolonged coadministration may cause systemic corticosteroid effects such as Cushing syndrome and adrenal suppression.

What this interaction means in plain words

Fluticasone intranasal and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Voriconazole inhibits CYP3A4-mediated first-pass and systemic metabolism of fluticasone.

What to do

For short courses, monitor for corticosteroid excess; for long-term use consider an alternative intranasal steroid less dependent on CYP3A4, such as beclomethasone. Monitor for Cushingoid features and adrenal suppression.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
