Summary
Mifepristone inhibits CYP3A4 and can increase systemic exposure to both salmeterol and inhaled fluticasone. Higher salmeterol levels may cause tachycardia, QT prolongation and hypokalemia, while higher fluticasone levels may cause systemic corticosteroid effects.
What this interaction means in plain words
Fluticasone-salmeterol and Mifepristone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluticasone-salmeterol belongs to Respiratory; Mifepristone belongs to Endocrine.
Why it happens
Inhibition of CYP3A4-mediated metabolism of salmeterol and fluticasone by mifepristone.
What to do
Monitor for cardiovascular effects, hypokalemia and signs of corticosteroid excess such as adrenal suppression. Consider an alternative inhaled regimen if long-term or high-dose mifepristone is needed.
What to ask your pharmacist
