Summary
Foscarnet can prolong the QT interval and cause hypokalemia, hypocalcemia and hypomagnesemia, which increase arrhythmia risk. Vilanterol can add to the hypokalemia and cardiac stimulation.
What this interaction means in plain words
Fluticasone-vilanterol and Foscarnet can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-vilanterol belongs to Respiratory; Foscarnet belongs to Anti Virals.
Why it happens
Additive QTc prolongation, aggravated by foscarnet-induced electrolyte disturbances and beta-agonist potassium lowering.
What to do
Monitor electrolytes closely and correct deficits, and obtain ECG monitoring during foscarnet therapy. Consider alternatives to the LABA if QT prolongation develops.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
