Summary
Systemic timolol blocks beta-2 receptors and can reduce or abolish the bronchodilation produced by vilanterol, with a risk of bronchospasm in patients with airway disease. Even ophthalmic timolol can be absorbed enough to cause bronchospasm in susceptible patients.
What this interaction means in plain words
Fluticasone-vilanterol and Timolol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-vilanterol belongs to Respiratory; Timolol belongs to Cardiovascular.
Why it happens
Pharmacodynamic antagonism through non-selective beta-receptor blockade.
What to do
Avoid non-selective beta-blockers, including timolol eye drops, in patients with asthma or COPD; use a non-beta-blocker glaucoma agent or a cardioselective beta-blocker if one is needed. Monitor respiratory symptoms if co-therapy continues.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
