Summary
Letermovir inhibits OATP1B1/3 and raises exposure of many statins, and fluvastatin concentrations may also increase. This raises the risk of myopathy and rhabdomyolysis, and use is not recommended when cyclosporine is also given.
What this interaction means in plain words
Fluvastatin and Letermovir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluvastatin belongs to Cardiovascular; Letermovir belongs to Anti Virals.
Why it happens
Letermovir inhibits OATP1B1/3 hepatic uptake and possibly BCRP, increasing fluvastatin exposure.
What to do
Consider reducing the fluvastatin dose and monitor closely for muscle symptoms. Avoid fluvastatin if the patient also receives cyclosporine.
