Summary
Letermovir can raise ergotamine exposure by inhibiting its CYP3A-mediated clearance. Higher ergotamine levels increase the risk of ergotism, with severe peripheral vasospasm and tissue ischemia.
What this interaction means in plain words
Ergotamine and Letermovir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ergotamine belongs to Central Nervous System; Letermovir belongs to Anti Virals.
Why it happens
Letermovir inhibits CYP3A4, reducing the metabolism of ergotamine.
What to do
Do not co-administer ergot alkaloids with letermovir; use a non-ergot agent for migraine during letermovir therapy.
