Summary

Mavacamten is metabolised largely by CYP2C19, which fluvoxamine strongly inhibits. Higher exposure can reduce ejection fraction and cause heart failure due to systolic dysfunction.

What this interaction means in plain words

Fluvoxamine and Mavacamten are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Fluvoxamine belongs to Central Nervous System; Mavacamten belongs to Cardiovascular.

Evidence level: High

Why it happens

Fluvoxamine inhibits CYP2C19 (and CYP3A4) and reduces mavacamten clearance.

What to do

Avoid the combination and select another antidepressant. If fluvoxamine is started or stopped in a patient on mavacamten, monitor LVEF and adjust the dose per labeling.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
