Summary

Fluvoxamine strongly inhibits CYP1A2, the main pathway of olanzapine clearance, and can substantially raise olanzapine concentrations. This may increase sedation, orthostatic hypotension, anticholinergic effects and metabolic or extrapyramidal adverse effects, with additive CNS depression.

What this interaction means in plain words

Fluvoxamine and Olanzapine / samidorphan can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Fluvoxamine inhibits CYP1A2-mediated olanzapine metabolism, with additional additive CNS depressant effects.

What to do

Start with a lower olanzapine dose and titrate according to response, monitoring sedation, blood pressure and tolerability. Reassess the dose if fluvoxamine is started or stopped.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
