Summary
Both formoterol and methadone can prolong the QTc interval, so using them together may add to the risk of torsades de pointes, particularly in patients with other risk factors such as hypokalemia or structural heart disease. Beta-agonists can also lower serum potassium, which further increases arrhythmic susceptibility.
What this interaction means in plain words
Formoterol-mometasone and Methadone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Formoterol-mometasone belongs to Respiratory; Methadone belongs to Musculoskeletal.
Why it happens
Additive QT prolongation through pharmacodynamic effects, with beta-agonist-induced hypokalemia as a contributing factor.
What to do
Obtain a baseline ECG and monitor QTc after starting or adjusting either drug, and correct potassium and magnesium abnormalities. Consider alternatives or dose reduction if QTc becomes markedly prolonged.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
