Summary
Fosaprepitant is converted to aprepitant, a moderate CYP3A4 inhibitor, which can raise exposure to corticosteroids that are CYP3A4 substrates. Hydrocortisone is cleared partly by CYP3A4, so systemic steroid effects may be modestly enhanced. The effect is usually small after a single dose of fosaprepitant.
What this interaction means in plain words
Fosaprepitant and Hydrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosaprepitant belongs to Gastrointestinal Tract; Hydrocortisone belongs to Endocrine.
Why it happens
Aprepitant, the active metabolite of fosaprepitant, inhibits CYP3A4 and reduces hydrocortisone clearance.
What to do
With repeated or prolonged hydrocortisone therapy, watch for excess glucocorticoid effects such as hyperglycemia, fluid retention and hypertension. Reduce the dose of oral or intravenous hydrocortisone if these appear.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
