Summary

Foscarnet is strongly nephrotoxic and causes electrolyte disturbances; combined with vancomycin the risk of acute kidney injury rises. Worsening renal function also increases vancomycin exposure.

What this interaction means in plain words

Foscarnet and Vancomycin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Foscarnet belongs to Anti Virals; Vancomycin belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Additive nephrotoxicity, with foscarnet causing tubular injury and crystal nephropathy.

What to do

Maintain hydration with saline and monitor creatinine, calcium, magnesium, potassium and phosphate frequently. Adjust both drugs to renal function and use vancomycin level monitoring.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
