Ibuprofen can reduce the blood pressure lowering effect of fosinopril and hydrochlorothiazide. With the ACE inhibitor and diuretic it increases the risk of acute kidney injury and hyperkalemia, especially in older or volume-depleted patients.
What this interaction means in plain words
Fosinopril-hydrochlorothiazide and Ibuprofen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fosinopril-hydrochlorothiazide belongs to Cardiovascular; Ibuprofen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and sodium excretion, opposing ACE inhibitor and thiazide actions.
What to do
Prefer paracetamol where suitable. If ibuprofen is needed, use the lowest dose for the shortest time, maintain hydration, and monitor blood pressure, creatinine and potassium.
What to ask your pharmacist
