Furosemide can increase metformin exposure and precipitate dehydration/AKI, increasing risk of metformin accumulation and lactic acidosis.
Mechanism of action
PK: ↑ metformin Cmax/AUC with furosemide; PD: diuresis → volume depletion/AKI → reduced metformin clearance.
Clinical management
Monitor renal function and glycemic control; counsel on hydration; consider temporary metformin hold during acute illness or aggressive diuresis; reassess eGFR with dose changes.
