Summary

Loop diuretics and aminoglycosides both injure the cochlea and kidney. Oral neomycin is poorly absorbed, but absorption can increase with damaged gut mucosa or renal impairment, so the risk of hearing loss and kidney injury is raised in those settings.

What this interaction means in plain words

Furosemide and Neomycin po can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive ototoxicity and nephrotoxicity, with loop diuretic volume depletion further raising aminoglycoside exposure and renal vulnerability.

What to do

Avoid the combination in patients with renal impairment, inflammatory bowel disease or prolonged therapy; otherwise monitor renal function and hearing and keep the patient well hydrated.
