Summary

NSAIDs such as parecoxib (via its active metabolite valdecoxib) can blunt the diuretic and antihypertensive effect of furosemide through inhibition of renal prostaglandins. In volume-depleted or at-risk patients they may also add to the risk of acute kidney injury.

What this interaction means in plain words

Furosemide and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

COX-2 inhibition reduces renal prostaglandin synthesis, causing sodium and water retention and decreased renal perfusion, which opposes loop diuretic action.

What to do

Monitor blood pressure, fluid status, weight and serum creatinine, and ensure adequate hydration. Use the lowest effective parecoxib dose for the shortest time and consider an alternative analgesic in heart failure or renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
