Summary
Adding an ACE inhibitor to a patient on a loop diuretic, particularly one who is volume depleted, can cause marked first-dose hypotension. Renal function may also decline because efferent arteriolar tone is reduced while renal perfusion is already compromised.
What this interaction means in plain words
Furosemide and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Diuretic-induced volume and sodium depletion activates the renin-angiotensin system, so ACE inhibition produces an exaggerated fall in blood pressure and glomerular filtration pressure.
What to do
Consider reducing or temporarily holding the furosemide dose before starting quinapril, and begin with a low quinapril dose. Monitor blood pressure, serum creatinine and potassium in the first one to two weeks.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Can I take Furosemide with Quinapril?
MediHelp rates the interaction as moderate. Furosemide and Quinapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken. Adding an ACE inhibitor to a patient on a loop diuretic, particularly one who is volume depleted, can cause marked first-dose hypotension. Renal function may also decline because efferent arteriolar tone is reduced while renal perfusion is already compromised.
How serious is the interaction between Furosemide and Quinapril?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Consider reducing or temporarily holding the furosemide dose before starting quinapril, and begin with a low quinapril dose. Monitor blood pressure, serum creatinine and potassium in the first one to two weeks.
