Summary

Combining a loop diuretic with a thiazide-like diuretic produces sequential nephron blockade and a markedly enhanced diuresis. This can cause profound volume depletion, hypokalemia, hyponatremia and renal impairment.

What this interaction means in plain words

Furosemide and Xipamide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Additive natriuresis from blocking sodium reabsorption at both the loop of Henle and distal convoluted tubule.

What to do

Monitor electrolytes, renal function, weight and blood pressure within the first days of combination therapy. Start with low doses and adjust or supplement potassium as needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
