Summary

A P-glycoprotein interaction between indinavir and gentamicin is theoretical, as gentamicin is cleared mainly by glomerular filtration. Indinavir can itself cause crystalluria, nephrolithiasis and renal impairment, which may reduce gentamicin clearance and add to kidney injury.

What this interaction means in plain words

Gentamicin and Indinavir can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Indinavir-related renal toxicity may reduce aminoglycoside clearance; P-glycoprotein effects are theoretical.

What to do

Monitor serum creatinine and gentamicin concentrations, and keep the patient well hydrated. Adjust the gentamicin dose if renal function declines.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
