Summary

Both aminoglycosides and magnesium impair neuromuscular transmission, so combined use can increase muscle weakness and, in severe cases, respiratory depression. The risk is greatest with parenteral magnesium, in renal impairment, or with neuromuscular disease. Gentamicin may also cause renal magnesium wasting.

What this interaction means in plain words

Gentamicin and Magnesium supplement can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: Moderate

Why it happens

Additive neuromuscular blockade through reduced presynaptic acetylcholine release and decreased postsynaptic sensitivity.

What to do

Avoid combining parenteral magnesium with gentamicin where possible, or monitor respiratory function and neuromuscular status closely. Check serum magnesium and adjust doses.
