Summary

Thiazide diuretics lower serum potassium and magnesium, and gentamicin can cause renal potassium and magnesium wasting. Together they may deepen electrolyte depletion, raising arrhythmia risk. Diuretic-induced volume depletion may also increase aminoglycoside nephrotoxicity.

What this interaction means in plain words

Gentamicin and Methyclothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Additive renal potassium and magnesium loss, plus diuretic-induced volume depletion that can raise aminoglycoside exposure and renal toxicity.

What to do

Monitor serum potassium, magnesium and renal function regularly, and replace electrolytes as needed. Keep the patient well hydrated and adjust gentamicin dosing using serum drug levels.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
