Summary

Parecoxib, like other NSAIDs, can reduce renal prostaglandin-mediated perfusion and impair kidney function. Gentamicin is nephrotoxic and renally cleared, so reduced clearance may raise aminoglycoside levels and increase the risk of kidney injury and ototoxicity.

What this interaction means in plain words

Gentamicin and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

NSAID-induced reduction in renal blood flow and glomerular filtration adds to direct aminoglycoside tubular toxicity and reduces gentamicin elimination.

What to do

Monitor serum creatinine and gentamicin trough levels, keep the patient well hydrated, and use the shortest possible parecoxib course. Consider a non-NSAID analgesic in patients with renal impairment.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
