Summary

Gentamicin is nephrotoxic, and with an ARB such as telmisartan, which reduces glomerular filtration pressure, the risk of acute kidney injury and electrolyte disturbance may increase. Gentamicin can also cause renal potassium and magnesium wasting.

What this interaction means in plain words

Gentamicin and Telmisartan-amlodipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Gentamicin belongs to Anti Bacterial; Telmisartan-amlodipine belongs to Cardiovascular.

Evidence level: Low

Why it happens

Aminoglycoside tubular toxicity combines with ARB-reduced glomerular perfusion, increasing renal injury risk.

What to do

Monitor creatinine, potassium and magnesium and gentamicin levels during therapy. Consider temporarily holding telmisartan if the patient is dehydrated or renal function worsens.

What to ask your pharmacist
