Summary
Gentamicin can cause renal injury, and angiotensin receptor blockers reduce glomerular perfusion pressure, which may increase the risk of acute kidney injury, especially with volume depletion. Resulting renal impairment can also raise aminoglycoside levels and increase toxicity, and telmisartan can cause hyperkalemia.
What this interaction means in plain words
Gentamicin and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gentamicin belongs to Anti Bacterial; Telmisartan belongs to Cardiovascular.
Why it happens
Telmisartan's hemodynamic effects on the kidney add to the direct tubular toxicity of gentamicin, reducing aminoglycoside clearance.
What to do
Monitor serum creatinine, potassium and gentamicin levels, and ensure adequate hydration. Consider temporarily withholding telmisartan if renal function declines.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Gentamicin with Telmisartan?
MediHelp rates the interaction as moderate. Gentamicin and Telmisartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Gentamicin and Telmisartan?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Monitor serum creatinine, potassium and gentamicin levels, and ensure adequate hydration. Consider temporarily withholding telmisartan if renal function declines.
