Summary

Glecaprevir and pibrentasvir inhibit P-glycoprotein and other transporters and may raise tacrolimus exposure. In addition, tacrolimus levels can shift as hepatic inflammation resolves during hepatitis C clearance, creating a risk of toxicity or under-immunosuppression.

What this interaction means in plain words

Glecaprevir / pibrentasvir and Tacrolimus can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of P-glycoprotein (and intestinal efflux/CYP3A4 effects) by glecaprevir/pibrentasvir increases tacrolimus absorption and exposure.

What to do

Measure tacrolimus trough levels before starting and frequently during and after hepatitis C treatment, adjusting the dose to target range. Monitor renal function and signs of tacrolimus toxicity.

What to ask your pharmacist

  • Should I take them at different times of day?
