Summary
The MMR vaccine contains live attenuated viruses. In tacrolimus-treated patients these viruses may replicate unchecked and cause vaccine-strain disease, and the immune response is likely to be poor.
What this interaction means in plain words
Measles mumps and rubella vaccine live and Tacrolimus are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Why it happens
Calcineurin inhibition suppresses cellular immunity, permitting replication of live attenuated vaccine viruses.
What to do
Do not give MMR vaccine to patients on tacrolimus. Vaccinate before transplant or immunosuppression where possible, and check immunity by serology.
