Summary

Mesalamine is a salicylate derivative, but at usual doses systemic exposure is low and salicylate-type effects on sulfonylureas are unlikely. Any enhancement of gliclazide's effect would be expected only with large systemic salicylate doses.

What this interaction means in plain words

Gliclazide and Mesalamine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Gliclazide belongs to Endocrine; Mesalamine belongs to Gastrointestinal Tract.

Evidence level: Low

Why it happens

Theoretical displacement of gliclazide from plasma proteins and additive glucose lowering by salicylate, which is limited with mesalamine.

What to do

No routine change is needed. Monitor blood glucose if high-dose mesalamine is used in a patient with poorly controlled diabetes.
