Rifapentine induces hepatic drug-metabolizing enzymes, which can lower glyburide exposure and weaken glycemic control. Rifapentine may also reduce metformin exposure to some extent. Blood glucose may rise during rifapentine therapy and fall again after it is stopped.

What this interaction means in plain words

Glyburide-metformin and Rifapentine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glyburide-metformin belongs to Endocrine; Rifapentine belongs to Anti Bacterial.

Evidence level: Moderate

Why it happens

Rifapentine induces CYP2C9 and other enzymes, increasing glyburide clearance.

What to do

Monitor blood glucose closely when rifapentine is started and increase the antidiabetic dose if needed. Reassess and reduce the dose after rifapentine is stopped to avoid hypoglycemia.
