Summary

Glycopyrronium blocks the muscarinic adverse effects of pyridostigmine such as salivation, cramping and diarrhea, and is sometimes used for that purpose. It does not impair the nicotinic benefit in myasthenia, but it may mask early signs of cholinergic excess.

What this interaction means in plain words

Glycopyrronium and Pyridostigmine have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Glycopyrronium belongs to Respiratory; Pyridostigmine belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Pyridostigmine inhibits acetylcholinesterase, whereas glycopyrronium blocks muscarinic receptors, so the muscarinic effects are antagonised.

What to do

Use deliberately where needed to control muscarinic effects, and monitor for muscle weakness that could reflect cholinergic crisis being masked.

What to ask your pharmacist
