Summary

Selumetinib is cleared largely by CYP3A4, and haloperidol is a weak to moderate CYP3A4 inhibitor. Coadministration may raise selumetinib exposure and increase its toxicities such as gastrointestinal effects, rash and cardiomyopathy. The effect is probably modest compared with strong inhibitors.

What this interaction means in plain words

Haloperidol and Selumetinib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Haloperidol belongs to Central Nervous System; Selumetinib belongs to Oncology.

Evidence level: Low

Why it happens

Haloperidol inhibits CYP3A4, decreasing the metabolism of selumetinib.

What to do

Monitor for selumetinib adverse effects and consider an alternative antipsychotic that does not inhibit CYP3A4. If a labeled CYP3A4 inhibitor dose reduction is required, follow the selumetinib labeling.

What to ask your pharmacist
