Summary
NSAIDs such as meclofenamate can reduce the antihypertensive and diuretic effects of thiazides and ACE inhibitors. The three-drug combination of NSAID, diuretic and ACE inhibitor raises the risk of acute kidney injury and hyperkalemia, especially with dehydration or existing renal impairment.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Meclofenamate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
NSAID inhibition of renal prostaglandins causes sodium and water retention and reduces renal perfusion, opposing diuretic and ACE inhibitor effects.
What to do
Use the lowest effective NSAID dose for the shortest time and maintain hydration. Monitor blood pressure, serum creatinine and potassium.
What to ask your pharmacist
