Summary
Thiazide diuretics and beta-2 agonists can each lower serum potassium. Together they may produce additive hypokalemia, which can predispose to arrhythmias, especially in patients with heart disease or hypoxia. The effect is mainly relevant at higher beta-agonist doses.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Olodaterol inhaled can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive hypokalemia: thiazide-induced renal potassium loss plus beta-2 agonist-driven intracellular potassium shift.
What to do
Check serum potassium and magnesium at baseline and periodically in patients on this combination, particularly those with cardiac disease. Replace potassium if low; the ACE inhibitor component may partly offset the loss.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
