Summary
NSAIDs such as parecoxib (via its active metabolite valdecoxib) can blunt the antihypertensive effect of ACE inhibitors and thiazides. Combined with an ACE inhibitor and diuretic, they raise the risk of acute kidney injury, particularly in volume-depleted or elderly patients, and can cause hyperkalemia.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Parecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Inhibition of renal prostaglandin synthesis causes sodium and fluid retention and reduces glomerular perfusion, which counteracts the drugs' effects and adds to renal hemodynamic stress.
What to do
Keep parecoxib to the shortest effective course with adequate hydration. Monitor blood pressure, serum creatinine and potassium, especially in the first weeks and in older or dehydrated patients.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
