Summary

Penbutolol is a beta-blocker that lowers blood pressure, and combining it with a thiazide and ACE inhibitor gives additive hypotension and possible bradycardia. Beta-blockade has only a minor effect on potassium, so any potassium interaction is of limited importance. Thiazides can also modestly impair glucose tolerance and mask the warning signs of hypoglycemia in diabetic patients on beta-blockers.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Penbutolol have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Evidence level: Moderate

Why it happens

Additive pharmacodynamic blood pressure lowering through different pathways; beta-blockade slightly impairs cellular potassium uptake.

What to do

Monitor blood pressure and heart rate when starting or adjusting doses. Check potassium periodically.
