Summary

Non-acetylated salicylates can have NSAID-like effects on the kidney, reducing the antihypertensive effect of ACE inhibitors and thiazides. With this combination the risk of renal impairment and hyperkalemia rises, particularly with high doses or volume depletion.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Salicylates can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of renal prostaglandin synthesis reduces renal perfusion and promotes sodium retention.

What to do

Use the lowest effective salicylate dose and ensure adequate hydration. Monitor blood pressure, creatinine and potassium.
