Summary
Bowel preparations cause fluid and electrolyte losses, and with a thiazide and an ACE inhibitor they raise the risk of dehydration, hypotension, hyponatremia and acute kidney injury. Electrolyte disturbances can also provoke arrhythmias.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Sodium sulfate / potassium chloride / magnesium sulfate / polyethylene glycol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Additive volume and electrolyte depletion from osmotic purgation and thiazide diuresis, with ACE inhibitor reduced renal perfusion tolerance.
What to do
Consider holding the diuretic and ACE inhibitor on the day of bowel preparation, maintain hydration and check electrolytes and renal function in high-risk patients.
