Summary
Terbutaline drives potassium into cells through beta-2 stimulation, and hydrochlorothiazide increases urinary potassium loss. Together they can produce additive hypokalemia, which may provoke arrhythmias, especially at higher terbutaline doses. The lisinopril component partly offsets potassium loss but does not remove the risk.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Terbutaline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Terbutaline belongs to Respiratory.
Why it happens
Additive hypokalemia from beta-2 mediated intracellular potassium shift plus thiazide-induced renal potassium wasting.
What to do
Monitor serum potassium, particularly when terbutaline is given at high doses or by infusion, and replace potassium as needed. Take extra care in patients with cardiac disease or other QT-prolonging drugs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
