Summary

Trilostane inhibits adrenal steroid synthesis, including aldosterone, which reduces potassium excretion. With an ACE inhibitor it can cause hyperkalemia, and added to a thiazide it may enhance hyponatremia and hypotension.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Trilostane can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Inhibition of 3-beta-hydroxysteroid dehydrogenase lowers aldosterone, adding to ACE inhibitor-induced potassium retention and sodium loss.

What to do

Monitor serum potassium, sodium and blood pressure when the combination is used, and adjust therapy if hyperkalemia or hypotension develops.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
