Summary
Xipamide is a thiazide-like diuretic, so adding it to hydrochlorothiazide duplicates therapy. The result is excessive diuresis with hypotension, hyponatremia, hypokalemia and renal impairment, which is increased further with the ACE inhibitor.
What this interaction means in plain words
Hydrochlorothiazide-lisinopril and Xipamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Why it happens
Additive inhibition of sodium chloride reabsorption in the distal tubule from two thiazide-type diuretics.
What to do
Avoid combining two thiazide-type diuretics. If sequential nephron blockade is intended, use a different class and monitor electrolytes and creatinine.
