Summary
Naproxen can reduce the antihypertensive effect of losartan and hydrochlorothiazide. Together with an ARB and a diuretic it raises the risk of acute kidney injury and hyperkalemia, especially in older or volume depleted patients.
What this interaction means in plain words
Hydrochlorothiazide-losartan and Naproxen can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-losartan belongs to Cardiovascular; Naproxen belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal blood flow and sodium excretion, opposing ARB and diuretic actions and impairing glomerular filtration.
What to do
Monitor blood pressure, serum creatinine and potassium within the first weeks of combined use and maintain hydration. Prefer paracetamol or the shortest possible NSAID course at the lowest dose.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
