Summary

Combining a loop diuretic with a thiazide gives synergistic diuresis, increasing the risk of volume depletion, hypotension, hypokalemia, hyponatremia and renal impairment. Olmesartan may partly offset potassium loss but adds to the risk of renal hypoperfusion.

What this interaction means in plain words

Hydrochlorothiazide-olmesartan and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydrochlorothiazide-olmesartan belongs to Cardiovascular; Torsemide belongs to Cardiovascular.

Evidence level: High

Why it happens

Sequential nephron blockade with additive diuretic effect and electrolyte loss, combined with ARB-related renal hemodynamic changes.

What to do

Monitor electrolytes, creatinine, weight and blood pressure, particularly in the first weeks. Reduce diuretic doses if volume depletion occurs.
