Summary
Valsartan reduces aldosterone-mediated potassium excretion and potassium citrate adds a potassium load, which can cause hyperkalemia, especially in renal impairment. Hydrochlorothiazide tends to lower potassium and may partly offset this, but the net effect is unpredictable.
What this interaction means in plain words
Hydrochlorothiazide-valsartan and Potassium citrate can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Hydrochlorothiazide-valsartan belongs to Cardiovascular; Potassium citrate belongs to Gastrointestinal Tract.
Why it happens
ARB-induced reduction of renal potassium excretion combined with exogenous potassium supplementation, partly opposed by thiazide kaliuresis.
What to do
Monitor serum potassium and renal function regularly after starting or changing doses. Reduce or stop potassium citrate if potassium rises.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
